The insurance provider will give your company a claim number and name of the adjuster - you’ll need this information to give your provider.
Scheduling an appointment
The first step is to select a provider to give you care - chiropractors can act as your attending physician, so you won’t have to go to anyone else first.
When you contact us to book an appointment, we’ll need to know the following things in addition to your basic contact info:
That this is a workplace injury
Your claim number & adjuster name
Before your appointment
Patients coming to us for workers’ compensation care will need to do some additional paperwork before their appointment. Please come in approximately 30 minutes early to finish the paperwork.
At your appointment
You’ll go through our basic new patient process but will have a slightly longer initial exam. We need to ask questions about the incident and get a detailed report of your injuries.
If you need an x-ray, we will provide you with a referral and a list of providers you can go to. Before you leave the appointment, we’ll schedule a follow on appointment for after your x-ray.
After the appointment
We will take care of billing your insurance, so all you have to do is rest and take care of yourself until the next appointment with us!
